I have been a healthcare professional for over 35 years, and have always seen myself as part nurse and part patient advocate. I try to make sure each patient’s rights are preserved, and that all patients receive the same high levels of professional care. Although there is usually an actual patient advocate position within the hospital or care facility, I believe all nurses would become more involved in the process if they could only see both sides of patient advocacy.
Put yourself in your patient’s position you were shopping, sleeping, playing with your children, or driving your car when you were suddenly stricken by a health crisis or accident. You have lost control of everything as you are whisked off to an emergency room and admitted to the hospital. In addition to your pain, you will probably feel overwhelmed and have a flurry of questions. In the middle of all this you may be visited by a patient advocate, a professional who works on behalf of patients and families during their hospital stay.
The patient advocate will probably give you a Patient’s Bill of Rights, a list of guarantees for those receiving medical care. In New York, there are nineteen factors listed in this Bill including the right to high quality hospital care, a clean and safe environment, involvement in your care, protection of your privacy, help when leaving the hospital, and help with your billing claims. The advocate will explain your rights and tell you what to do if you feel your rights have been violated.
After the advocate leaves, though, you may not remember everything that was said. That’s where a second tier of patient advocacy is necessary. Other health care professionals, like doctors, nurses, and social workers can also serve as patient advocates. In fact, nurses make great patient advocates. Their extensive educational background in health care often imbues them with a plethora of information they can use to advocate for higher levels of patient care.
Most hospital patients can benefit from the services of a patient advocate. If any issue arises with the hospital or care they are receiving, patients may become confused or frustrated. Many times, nurses can serve as an advocate to help settle the issue, since they are with the patients most of the time. Nurses are very knowledgeable about the patient bill of rights, the hospital, and their patients, and can often anticipate patient needs before others.
Looking at Patient Advocacy from the Other Side
During my healthcare profession, I often served as a patient advocate, either knowingly or sub-consciously while performing my routine duties. But I never realized how important a patient advocate can be until I became a patient myself.
Prior to last year I was never in a hospital, never had a surgery, and never struggled with any long illness. I did have fibroids and other minor issues, but it never occurred to me that something major was happening when I began exhibiting any real symptoms. Even though I am a nurse, I thought my pain was due to the flu or that I had pulled a muscle, until I was diagnosed with ovarian cancer.
My first advocate was the doctor was gave me this unsettling news. When I protested that I had never been sick before, he helped me understand that I was now a patient who would need to rely on others. After tests to determine the type and extent of the cancer, I was scheduled for my first major surgery and first stay in a hospital. Soon I was on the other side of the bed rail, asking other medical professionals for help. It felt strange, but I also experienced a new sense of appreciation for my profession. I stayed in the hospital for five days, and was discharged on one of those brutally cold Buffalo days when the temperature was only 3°. Although the cancer had not metastasized, I underwent a course of chemotherapy at Roswell Park Cancer Institute.
My first session did not go well, as I had an almost immediate reaction to the drugs. That’s where I met more advocates on the nursing team, who were quick to respond to my needs. Although I remain confident that the drug regimen is doing its job, recent tests showed that my hemoglobin was low, another small tumor had appeared, and there was a clot in my lung. I received three units of blood, and was advised to stay overnight for further care. Being a stubborn nurse, I was confident I could go home and take care of myself. That’s where another advocate took over, this time in the form of an intern who advised me to stop being so mule-headed and to let the medical team do its job.
When I return to work, I think this experience will make me a better nurse. Although I always treated patients with kindness and respect, I realize how important the nurse is in the patient advocacy process. I am a nursing teacher, but I still learned from the medical professionals I encountered. I have a much deeper understanding of the impact nurses have on every patient.
All in all, I believe that the number one lesson of patient advocacy is being open to giving and receiving care. Patients need to know they don’t have to face their illness alone.
