A seasoned nurse you say…what exactly is a seasoned nurse? An old nurse…no, a nurse that knows everything about everything…no, a nurse that has years of experience in a particular area of the profession and has become an expert in that area…yes! This, readers, is achieved through a phenomenon dubbed in the professional realm as, deliberate practice. Deliberate practice is an activity that is not fun but rather difficult, that several nurses do while not realizing it. For example, nursing students are shown a task in the skills or simulation lab and then sent off to practice that skill. Let us take intravenous (IV) starts for example. This can be a fun but challenging skill for some however, with practice and instructor supervision, one can learn to perform well. Understanding the difference between the fun of learning such a task and the mindful learning associated with deliberate practice is the key to great performance. The learning associated with any skill does not stop in the skills lab, however, but rather continues along as the student progresses through school, graduates, and becomes an employed nurse. I will use myself as an example. Once I became a Labor & Delivery nurse, I truly needed to step up my game of starting IVs successfully. I did this through mindful learning and continuous practice to move toward automaticity or mastery level of successful IV starting. While some may view success as a talent (or lack thereof in some cases), nurses become experts through deliberate practice to improve performance…the more we do, the better our performance.
So…back to the topic of “seasoned nurse”. As mentioned earlier, the seasoned nurse is now known as, according to Benner’s theory, an expert in his or her area of expertise. This could be ICU, NICU, ER, Med-Surg, or many other areas of nursing. Does this mean he/she has nothing left to learn, absolutely not? As nurses move through their profession, they develop knowledge, skills, and attitudes and become masters in their “comfort zone”. If you are familiar with Benner’s theory (side note…if you are not, you should be!), graduate nurses start out in the beginner stage which is known as the Novice stage. Nursing programs are built to provide graduate nurses with a solid foundation of the basics of the profession. Withholding a solid foundation in a nursing program would be a semblance to building a costly structure on a base of beach sand where it would take just one critical moment that resulted in a sentinel or catastrophic event. We do not want any of those so nurse educators pride ourselves on providing such building blocks and role modeling in this wonderful profession. In case you have not guessed, I am a nurse educator and I absolutely love this part of my nursing profession.
Another concept that new grads will be introduced to is floating. In Part I, I discussed briefly, the nursing shortage…well, it still exists so occasionally when one facility unit has a low patient census, another may have a high patient census…thus the term floating or practicing on another unit from your home unit. Many times, in our careers as nurses, we will find ourselves being floated to another unit. Well, when you take a seasoned nurse in one area and float them to another, guess what? They may very well become somewhat frazzled due to the new environment. Here I bring back the penny theory discussed in part I. A nice, tarnished penny or seasoned nurse has been floated to an area of unknown or unfamiliarity. Is this penny shiny and new again or does it remain tarnished? Well, according to my theory, this penny has become somewhat new again. The nurse/penny still has those years of experience but has been put into an unfamiliar environment and must learn to adapt. Uncomfortable position yes, however, this process is practiced in many facilities today and nurses must learn to adapt to any given situation…after all, we have the basics of nursing down…or we should have!
Do you remember that five letter nasty word in nursing school that everybody said they would embrace kindly, but nobody wants to really do it? Change…oh dear! Change is inevitable in nursing folks. Every day, this profession changes. Now…do you learn to accept this positively or negatively…that is all up to you and how you see your future, but I would certainly hope you would accept it with open arms because change has brought this profession so many wonderful things.
