It was 1995, early in my nursing career. I was working at St. Vincent’s Hospital and Medical Center‐ the 8 am to 8 pm shift. At about 7:45 PM I had prepared an I.V. Piggy‐Back for one of my patients, and I was ready to hang it. Upon entering the room, I noticed my patient’s hands grasping at the fringes of her blanket, she also appeared slightly diaphoretic.
I decided to take another set of vital signs. Other than a slightly elevated B/P, all other vitals were WNL. What I hadn’t known at that instant was that she had just been visited by a young staff intern. The patient’s demeanor had changed; it seemed from one minute to the next. She was obviously anxious, and it showed.
The detective‐ nurse in me took over. I asked the patient if she would mind if I pulled up a chair, to which she replied “Of course not!”
I began, “I could not help but notice that you don’t’ seem to be your usual bubbly self, is there something upsetting you in any way?”
She replied, “Well actually, yes there is something, but I didn’t want to bother anyone.”
I said, “If you feel comfortable telling me, you are more than welcome.”
She said “The doctor told me that I was going to have a HIDA scan in the morning and that I can’t eat anything after 12 am. I don’t know what this scan is and why I can’t eat anything after midnight.” I began to explain what a HIDA scan is and the reason for having an empty stomach.
Well, my patient was extremely thankful and began to joke and smile again. She thanked me for taking the time to explain the procedure. I let her know that a thanks is not necessary, but is appreciated.
When I left my patient’s room several things were accomplished.
- My patient’s fear/anxiety was alleviated
- A Teaching moment was seized
- I as a nurse felt like I’ve done my job
The moral being: Sometimes postponing what has to be done, and doing something that needs to be done yields a greater patient benefit. My patients’ smile was worth, now well worth the ten minutes I took. I would gladly do it again! P.S. The I.V. piggy‐back went up at 8:03 PM not bad!
