Background and Significance:
Amiodarone is an important drug in combating dangerous and potentially fatal arrhythmias, but, when infused peripherally, it is particularly prone to damaging veins by vesicant extravasation.1 The spectrum of harm, from mild to severe, includes infiltration or extravasation, phlebitis, thrombophlebitis, necrosis, sepsis, and, potentially, death.2 Outcomes include pain, needed hyaluronidase treatment or plastic surgery, increased length of hospital and intensive care unit stays, and increased cost.1 Nurses are in a position to minimize these concerns by maximizing best practices in their care.
The low pH of amiodarone, from 3.5 to 4.5 creates a caustic environment.3 When in contact with di-(2-ethylhexyl)phthalate (DEHP) plasticizing agents in IV tubing, amiodarone precipitates into needle-like crystals, causing localized vein irritation and trauma2. The likelihood of precipitation affecting the tissue depends on administered “volume fraction”, calculated as a ratio of infused drug volume to blood flow volume. The lower this ratio, the lower the chance of precipitation.4
A literature search indicates that peripheral IV amiodarone results in an incidence of vein damage from 8% to 85%,2,3,4,5,6,7,8,9,10,11 with a weighted average of 18.3%. Recognizing this unacceptably high level, staff nurses on a vascular care floor at a midwestern teaching hospital determined to generate evidence-based change, reducing amiodarone associated phlebitis by 50%.
Specific Aim:
A PICO question was formed: For adult patients treated with peripherally infused amiodarone, can phlebitis be reduced significantly solely by adhering to maximal protective elements within the scope of nursing practice? A literature search identified nurse-controlled standards, from IV site care to the infusion. The study was aimed at phlebitis resulting from 24-hour infusions, usually for atrial fibrillation, atrial flutter, and supraventricular tachycardia12,13, rather than for emergency use.
Study Cohort:
The baseline cohort included a convenience sample of 73 patients discharged from February 2017 through March 2018. Data were collected before announcing the study to avoid influencing a change in nursing practice. The incidence of phlebitis in this group was 46.6%. To utilize an alpha of 0.05 with a power of 0.80, with a binomial endpoint and a goal of decreasing phlebitis by 50%, a target of 65 subjects in each cohort was set. The intervention cohort comprised 65 subjects, discharged from April 2018 to January 2019.
Methods:
This study describes a multi-dimensional approach to minimize amiodarone related phlebitis. All components of the intervention fall within the scope of nursing practice, requiring no outside regulation or approval. Following data collection on the baseline cohort, an education component was presented to all nurses on the vascular care unit, including the rationale behind each intervention. Practices gleaned from a literature search to decrease phlebitis constituted an intervention termed “Best Bundle Practices”, which was applied to the second cohort. The endpoint was the presence or absence of phlebitis in the peripheral vein in which amiodarone was infused. All areas affecting the potential for infiltration or irritation were addressed: the IV site, its security, the IV delivery system, and the volume fraction of the medication as it infused.
With few exceptions, amiodarone infusions in both cohorts followed the manufacturer-recommended protocol. This includes a 150 mg amiodarone bolus in 100 ml D5W (10 mg/ml) over 10 minutes (600 ml/hour), followed by amiodarone 900 mg in 500 ml D5W (1.8 mg/ml), infusing at 1 mg/minute (33.3 ml/hour) for 6 hours, and then 0.5 mg/minute (16.7 ml/hour) for 18 hours.14 Infusion duration varied, depending on the severity and refractory nature of the dysrhythmia, pre-medication with other antiarrhythmic agents, or incidence of hypotension.
The Infusion Nurses Society recommends that amiodarone be infused in a dedicated line with a 0.22-micron filter next to the peripheral cannula.15 These precautions were followed in the baseline cohort. Nurses also assessed IVs with nurse-to-nurse handoffs and every 4 hours with an infusing IV. Non-DEHP tubing was not available.
Interventions:
The Best Practices Bundle protocol included the above practices, and introduced the following interventions, along with supporting evidence:
1) To decrease contact with an irritating or caustic substance or formation of a precipitate within the vein, the use of non-DEHP tubing was added to the protocol.14
2) Slim et al. demonstrated the occurrence of increased cellular damage at concentrations of amiodarone of 2 mg/ml or higher.4 Recognizing this, amiodarone manufacturers recommend infusion by central venous catheter for higher concentrations, thereby decreasing the volume fraction of amiodarone.14 Though suboptimal, amiodarone frequently is administered peripherally.
Spiering produced a set of guidelines for amiodarone infusion. She demonstrates a change in phlebitis from 85% to 38%, a 55.2% reduction in incidence, utilizing a small sample, with n =34 in both the before and after samples. Spiering also attempted to utilize a more dilute infusion of amiodarone but was unable to obtain this from pharmacy.6 Once considered incompatible with isotonic saline, (NS) subsequent research has shown amiodarone and NS to be compatible.16 Nurses’ responses to a nursing blog indicated a diminishing incidence of phlebitis when infusing NS in a Y-port with amiodarone.17
To decrease the volume fraction, a second intervention involved dilution of amiodarone by nearly 50% by utilizing a co-infusion of NS in a Y-port nearest the infusion site. Because the subjects were cardiac patients, hypervolemia was a concern. In addition, the Infusion Nurses Society reports that IV infusions at a rate exceeding 50 to 90 ml/hr result in increased rates of phlebitis.15 Therefore, NS was infused at 25 ml/hr with the bolus and for the first 6 hours of the 24-hour infusion, and reduced to 15 ml/hr for the remaining 18 hours, totaling 420 ml in 24 hours. The new amiodarone concentration, near 1 mg/ml instead of 1.8 mg/ml, effectively lowered the volume fraction, and the higher pH of NS ameliorated the lower pH of amiodarone.4,17,18
3) To minimize phlebitis, the optimally sized catheter was placed in the optimal site. The Infusion Nurses Society recommends using the smallest possible cannula in a relatively large vein.15 An 18 or 20-gauge cannula was encouraged for maximal stability and minimal vein contact. Although a new IV was considered ideal, an analysis of IV age showed no positive correlation with the incidence of phlebitis. Assessment of selected IV for excellent condition and placement was preferred over the automatic placement of a new IV.
4) Nurses were asked to prioritize the use of an IV placed in a non-jointed area, as recommended by the Infusion Nurses Society, to alleviate the danger of cannula displacement or damage by flexion.15.
5) The Infusion Nurses Society recommends increased observation of IV sites dependent on the danger posed by extravasation.15 To decrease the severity of phlebitis should it occur, observation of the IV site was increased from every 4 hours to every 2 hours.
Statistical Analysis:
Data was entered into an Excel (Microsoft Corporation) spreadsheet and imported into SAS v9.4 (SAS Institute Inc.) for analysis. Discrete dichotomous endpoints were used, i.e., presence or absence of any injury to the vein. P-values were calculated using a chi-square test.
Results:
In the baseline cohort, 46.6% (n =34) of subjects demonstrated amiodarone related phlebitis. In contrast, the intervention group showed an incidence of 24.6% (n = 16). This represents a decrease of 47.2% with the intervention applied. The P-value = 0.006.
Conclusion:
The intervention showed both clinically and statistically significant results, indicating that attention to best nursing practices alone can have a marked impact on the reduction of peripherally infused amiodarone related phlebitis. While no single practice can be singled out as most effective, it appears that a more dilute solution plus the use of non-DEHP tubing offer promise as new standards of care.
The results are consistent with the findings of Spiering6 in a similar study, though this is a larger study and includes a co-infusion of isotonic saline. Should repeated research confirm these results, it also might be possible to convince manufacturers to provide an alternate concentration of amiodarone.
Limitations:
This represents a small study in a fairly homogenous, primarily Caucasian population. Further testing needs to be done to ascertain the reproducibility of results in a variety of populations. Additionally, utilization of separate portions of the Best Bundle protocol may serve to tease out which elements offer the greatest benefit and to determine whether all elements are necessary.
