Being a nurse for so many years and seeing all the changes in the environment of nursing is quite eye-opening. When I was taught, many moons ago, the most important part of nursing next to therapies and medications was giving quality care to the patients.
What Is Quality Care?
Taking care of all parts of the patient. Absolutely necessary is to Include their socio-economic, thoughts, intelligence, abilities, concepts of medicine, experiences, and effects of their illness on their ability to absorb information. Without taking all of those things into consideration, care will be compromised. Some people despise medical people and hospitals and are skeptical of what they are being told. Many patients are non-compliant because when they are told to do something, take medicine a certain way, or follow a regimen, they don’t do it because it isn’t explained to them the Why of the course of action.
Over and over I have seen non-compliance happening with patients because someone told the patient to do XYZ but offered no explanation to the patient as to how it would affect them directly. Talking to patients and finding out what is most important to them gives one a much better base to be able to give them what they need. Being able to adapt your patient care by including them in their care protocol helps so much to increase patient safety, healing and understanding.
Some simple things that make a huge difference for the patient are just being aware of simple things like looking at the alignment of the patient in the bed and correcting it with a pillow or straightening a blanket, Sometimes the patient doesn’t know what to do to make it more comfortable. These situations also give you a chance to double-check the patient’s skin integrity and awareness of other pain spots or missed dermal issues. So it increases your chance for observation and recognition of patient well-being.
One of the things I see missing as well is the education of the patient. Asking pertinent questions so you can find out what they know and understand about their illness is a great first place to start. Talking to the patient, yes it takes a bit of time, but the result in the healing experience is remarkable. Taking time and giving some time to the patient helps you to focus on what will make them feel cared for as well as finding out their thoughts about their illness, how it is affecting their home life and basic functioning.
The overseeing bodies that evaluate hospitals have instituted all kinds of rules, extra boxes to check, for things including care, education of the patient as well as monitoring of all their tests, physical care given, bodily functions, etc…so that there is more to do on the computer and nurses feel strained for time to make sure every box is checked and every line filled in as opposed to spending time with the patient.
The staffing of the floors also has had quite the effect of limiting quality time with patients at the bedside. I was working in a hospital one time in which the medications, iv’s, and procedures took the whole night to take care of. Then to try to chart them all in an expedient fashion was immeasurably difficult. When one would try to add assessments and other parts of care it often becomes overwhelming. Inevitably, if one would stop to deal with an issue with a patient, it would make one late out of work, for the charting would have to be done to ensure everything you did was there.
These situations have definitely decreased nurses’ ability to make time for their patients. It affects what patients truly know when they go home. I learned nursing theory under a wonderful theorist, Orem. Orem’s premise in nursing was to teach the patient better how to take care of themselves and it would protect them and limit the amount of care they would require in a hospital or medical setting. Education is necessary for all of our patients, but the restrictions of tasks, medications, staffing ratios, and stress levels have certainly all affected a nurse’s ability to give the best care, quality care to our patients.
